  • One page for each question
  • Between 2 and 5 answers for each question (in multiple choice format)
  • Suitable for skills tests and academic tests

  • For example: Never, Rarely, Sometimes, Often, Always

    • All questions on the same page
    • A rating scale with flexible / different custom headings for answers to each question

    • As another example in this style, see sample test for Diet Screening with Yes or No answers for some questions, while other questions have more complex answer choices such as Never, Once a Day, Once a Week, etc.

    • A rating scale with flexible / different custom headings for answers to each question - similar to Test Style 4 - but with an optional column for user comments (selectable for some questions but not all)
